Making a referral
What to do to make a referral to The Haven
We actively encourage people to make contact with The Haven themselves but we know that sometimes that isn’t easy for folk to do, especially the first time. So if it’s appropriate, we are happy for you to pass on your client/patient’s contact details to us and we’ll make contact with them, providing the appropriate GDPR consent has been gained beforehand of course!
We want to make sure that those who need Haven support can access it as quickly as possible, so we have made it really easy for you to make a referral to any of our services.
If you have a client/patient who you think may benefit from Haven support all you need to do is just pick up the phone and speak to a member of the team or alternatively send us an email. It’s as simple as that! No paperwork to complete at all! If you are unsure if The Haven is the most appropriate service for your client, again all you need to do is call us. We are happy to discuss each individual case with you.
Once we’ve received the contact details of a person requesting Haven support, one of our Practitioners will make contact with them via phone or email, to arrange a suitable time for an initial conversation. We’ll ask them about their needs and how best The Haven can support them. We aim to do this within 1 working day of receiving their information.
Working together for our communities
It’s really important for us to work alongside our Health and Social Care, Voluntary Sector, Education and Community colleagues to make sure that people living in the areas we serve can access and receive the most appropriate level of support when they need it most.
To do this it helps us if we know about the services you have available for people and the ways you work. We want to make sure that you know more about us too and what services are available at The Haven. We really like it when we get the chance to speak to others teams working in the community, to share and learn together. It’s always good to be able to put a face to a name!
